The two of you are married or, if not officially "married, are residing together and are considered to be a "couple". |

$1.69 per day membership fee
This is for one Senior AND their selected spokesperson.
You do not meet the Membership Type of "JOINT". |

Check your Eligibility
You are eligible to be an Ally@Home member if:
1) You are an INDIVIDUAL Membership applicant 60 years old or older at the time you submit the membership application; OR
2) At least one of the two of the persons submitting the membership application for a JOINT Membership Type is 60 years old or older at the time you submit the membership application; AND
3) The Senior member applicant(s) reside in the United States. |

Determine who is to be your Spokesperson
In addition to the one-person (i.e. INDIVIDUAL) or two-person (i.e. JOINT) membership types, you are allowed to name one other person who may act as your spokesperson who may phone Ally@Home for services on your behalf. That "spokesperson" is usually an adult child of the Senior member, but could also be a personal aide, friend, or neighbor. You will specify this person's contact information on your membership application. You are not required to name a spokesperson. If you do not name a spokesperson at this time, you may add a spokesperson at any future time by contacting Ally@Home with their contact information. |

Write a Check for your Membership Fee Payment
- We accept only a check for payment. No cash; no credit card.
- You must pay in full for one year's (i.e. 12 months) membership. At any time you may cancel your membership, at which time we will immediately mail your refund to you for all unused months.
- For an INDIVIDUAL membership, your check amount is $616.85
($1.69/day x 365 days/year = $616.85)
- For a JOINT membership, your check amount is $981.85
($2.69/day x 365 days/year = $981.85)

Mail your Membership Application and Check
Mail your completed Membership Application with your check for 12 months membership to;

We phone you to welcome you as an Ally@Home member
Immediately upon our receipt of your mailed membership application and check for payment, we will phone you to welcome you as an Ally@Home member upon which you may begin to use your Ally@Home. |
